That appears to be an EVO II lever from Speedycom Performance LTD in the UK.But they seem to be available nearly everywhere with anyone's logo on them....

Those look to be the direct replacement.Do you suppose they're the actual manufacturer?
I found them on an ebay ad from misano-moto-imports-mc, and that's the custom engraving arm it seems.
